Think about what you said:

*On startup/cold motor, the Coolant temp sensor tells the ECU and the ECU is programmed to run rich to compensate for a cold motor.*

as well:

"Turn on the a/c add it smoothes more but idles high"

*AC is a heavy drag on the motor and to compensate the motor runs richer/the AAC increases airflow to let the car run higher*


When my car had the shitty tune after doing big injectors it would start fine cold *richer*, but I couldn't start it warm.(It used the leaner warm-start map)
I figured it out by using Nissan Datascan and faking my engine being cold, worked like a charm *every* time.

So the question is, why are you running lean?
You put in a new IACV and AR when it started doing this and it changed nothing, right?

Did you check the connectors for them?
Have you tried checking the resistance of the wire itself from the IACV and the ECU? (Could have a short or be messed up)

Other than that, I might suspect a boost leak.
When it's cold does it feel like you're running on 5? Do a power balance test?
Fuel filter?

How about the timing? If set improperly it could be a problem after reaching operating temp.
